Registration 2018-2019
 

Dear Parents,

If you were able to complete online registration and/or Kindergarten Enrollment this past month, you will not need to attend Registration on the following dates. Please know that teacher assignments and calendars will not be distributed at this Registration. After August 1st, you will be able to

find out your child’s teacher in Family Web Access or by calling the school. All calendars will be distributed at your school’s Open House in August.

For families that did not register online:

The Alton Community Unit School District #11 will conduct the mandatory annual registration of returning elementary, middle school and incoming Kindergarten students at Alton High School Commons on the following dates.

This schedule is provided to assist with your planning.  If you can’t make your suggested time or have more than one student with different last names, please come at any of the times listed during these three days. High School Registration is July 30 - August 2, 11:00 p.m. - 7:00 p.m. each day.

 

NOTE: PARENT/GUARDIAN MUST SHOW CURRENT ID. IF PARENT/GUARDIAN IS UNAVAILABLE, INFORMATION MAY BE RELEASED TO AN EMERGENCY CONTACT PERSON ON RECORD WITH PROPER IDENTIFICATION.

 

Registration Stations

 

Station 1:  Registration Forms Parents/Guardians of returning students will show current photo identification and two proofs of residency to pick up the Registration Form needed at all other stations.

 

PROOF OF RESIDENCY: REQUIRED FOR ALL ELEMENTARY AND MIDDLE SCHOOL STUDENTS.

These proofs must be current and in the parent/guardian’s name with the enrolling address printed on it.

* Paycheck/Stub (if your address is imprinted on it)

* Major Credit Card Statement (not random offers to apply)

* Lease/Rental Agreement (dated and signed) * FOID card

* Installment Loan Contract from bank *Current Vehicle Emissions Notice

* Checking or Savings account statement * Insurance Policy for Home or Health

* Vehicle Registration Card *Resident service contract (appliance repair, exterminator, window installation)

No general mail will be accepted for registration.


Station 2:  Online Registration
Families will log in to Family Web Access to verify all information and make any changes necessary.

 

Station 3:  School Meals/Fee Waivers  Breakfast and Lunch will again be provided at no charge to enrolled students in Kindergarten - 12th grades.  Household Income forms will be available at this station for students that have not been Directly Certified for receiving SNAP or TANF benefits and will be used to determine if you qualify for a waiver of supplemental fees.  Be sure to bring at least 1 month's proof of income for everyone in the household or a print out from the DHS office showing that someone other than the student at the home address receives SNAP or TANF benefits.

 

Station 4:  Transportation You must stop at this station to verify how your student will get to and from school each day.

 

Station 5:  Fee Payment/Food Service Payment  The supplemental fee for the 2018-2019 school year will be paid at this station.

Current Fees:  Kindergarten – Grade 5 - $90.00 per year

Grade 6-12 - $100.00 per year.  Unified Arts – Grade 7 & 8 - $10.00

 

Station 6: School Stations Parents will turn in any necessary forms and confirm completion of the registration process at the child’s school.

Volunteer Form/ Waiver of Liability Form, Physical/Medical Forms/ USDA Food Allergies Documentations are all available at the school station. Physical, Dental, and Eye examination forms along with corresponding waivers are available upon request and also available on our website.  Any student who has special nutritional or feeding needs such as dietary restrictions or allergies MUST have a parent submitted USDA Form.

 

All efforts are put into making our K-8 district-wide registration a quick, efficient and pleasant experience for you.  Expect delayed wait times at the individual schools should you not attend the district-wide registration.

 

All Residency Affidavits/Guardianship requests and Special Education intakes will be conducted in the Library. Students new to the district will also start in the Library instead of Station 1, and then proceed to Station 2.

 

School will resume on August 20, 2018.  This will be a FULL DAY of attendance for students. Registration must be completed prior to attendance.

 

  • Kindergarten students will need updated immunization records, lead screening, school physicals and eye exam by an optometrist by the first day of school.
  • Kindergarten, 2nd and 6th grade students will need to have a dental check up by May 15th of the 2018-2019 school year.
